If you have a form you already use, we can codify the form into our database within a few days. You can combine this form with industry standards or other practitioner's forms.
Patients can access your forms on our hardware system in your office or over the Internet from their homes. Either way, data is stored securely in our database which is HIPAA compliant.
Because data can be accessed over the Internet, you are free to partner with an unlimited number of other providers. Across the office or around the world, providers can have their patients complete identical sets of forms and obtain a critical mass of patients quickly.
